Data controller
Dr Léo Prud'homme, whose details appear in the legal notice, is the controller for data processed via this site.
Data collected
The contact form collects your name, e-mail address and message, for the sole purpose of answering your request. Do not include medical information: such matters belong in a consultation.
Appointments are booked on Doctolib, which applies its own privacy policy.
Legal basis and retention period
This data is processed on the basis of your voluntary request (Article 6.1.b GDPR) and kept for a maximum of 12 months after the last exchange, then deleted.
Recipients
Messages are forwarded to Dr Prud'homme and his secretariat via the host's form service (Netlify). They are neither sold nor used for commercial purposes.
Cookies
This site uses no advertising cookies and no audience-measurement tools. Only technical session storage is used (to display the home animation); it contains no personal data and disappears when the browser is closed.
Your rights
Under the GDPR, you have rights of access, rectification, erasure and objection over your data. To exercise them: secretariat@ophtalife.com. You may also contact the French authority, the CNIL (cnil.fr).
