Exophthalmos (bulging eye)

Exophthalmos is the abnormal protrusion of the eye out of the orbit: the eye seems to "bulge" or look bigger. It is not a disease but a symptom, whose cause must be identified.

Recognising it

An eye more "forward" than the other or than in the past — comparing with old photos is often useful —, eyelid retraction, more visible white of the eye. It can affect one eye (unilateral) or both.

The causes

The most frequent in adults is thyroid eye disease (Graves' disease), usually bilateral. A unilateral or rapidly progressive exophthalmos should prompt a search for another orbital cause — inflammation, infection (orbital cellulitis), tumour or vascular anomaly — and a prompt consultation.

The work-up

Eye examination with exophthalmometry (measurement of the protrusion), thyroid tests, and orbital imaging (CT or MRI) depending on the context.
